All applications and authorizations must be received and approved prior to participating in any event.
We strongly encourage parent involvement; however, background checks may be required for certain events and outings.
Each student must be registered in our program as a Club member. This will entitle them to attend field trips and other educational learning activities the we offer.
This form is necessary for the staff at SCA and chaperones to keep on file for each child participating in the program's events.
If you agree to have your child's picture taken and displayed from any of our events, please fill out this form for our records. Pictures may be used on our social media sites or other printed materials.

Our chaperones are loving, supportive, and caring of our youth. If you are a parent or volunteer who would like to serve as a Chaperone at any of our events, please read and complete the attached agreement for review.
For the safety of your child, it is necessary for us to have a legal custody form for our records on each child participating in our program. Children will be released only to those listed as an authorized custodian.
Given the nature of our program being faith based, we want parents to understand the roles and responsibilities we have for your children and our expectations of them. Please read and complete the agreement form here.
Medical history forms are needed so that we can be aware and alert of any issues that may arise while your student is in our care. Be sure to list any allergies we need to be aware of.

This is a quick access form that is kept with our records at each event.
